Advanced Nurse Practitioner (NP or PA)

Job Summary
We are seeking a dedicated and skilled Advanced Practice Provider (Board Certified Nurse Practitioner or Certified Physician Assistant) to join our healthcare team. The ideal candidate will play a crucial role in delivering high-quality patient care in a post-acute care setting. 

Duties

• Conduct thorough patient assessments and develop individualized care plans.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Administer treatments and procedures as per established protocols. 
• Maintain accurate medical records and documentation in compliance with medical terminology standards.
• Stay updated on the latest advancements in physiology knowledge and trauma care practices.
• Participate in quality improvement initiatives and contribute to the development of best practices within the facility.

Experience

• Previous experience in skilled nursing, long term care and rehab
• Active New York license as a Nurse Practitioner or Physician Assistant
