Position Summary:
We have an exciting opportunity to join our team as a Advanced Fetal Care Center Nurse Coordinator.

In this role, the successful candidate Under the direction of the Program Director of Childrens Surgical Services, the Advanced Fetal Care Center (AFCC) Nurse Coordinator is an important partner to the Director of the AFCC at NYU Langone Health and is responsible for coordinating all aspects of fetal surgery care. The coordinator is responsible for coordinating the patients initial referral, pre-consult, initial consultation and post-delivery care. They should possess the ability to serve pregnant women, mothers and their family as they navigate their delivery journey. This person will have the interpersonal skills to build and cultivate strong relationships with referring physicians and members of the multidisciplinary care team that will include maternal fetal medicine specialists, obstetricians, fetal surgeons, pediatric surgeons, social workers, radiologists, cardiologists and genetic counselors. They will serve as a primary clinical liaison for the administrative infrastructure of the Advanced Fetal Care Center, partnering with the Director to identify growth opportunities within the center, and helping direct administrative priorities to ensure continuity between programmatic goals and clinical operations. They will also be responsible for coordinating ongoing care of obstetrical patients appointment scheduling, coordination with multiple specialties based on diagnoses and ongoing family support. They will also be responsible for collecting clinical information about patients, summarizing medical data, and presenting information to the fetal surgery team, arranging diagnostic testing and treatment procedures for patients. Additionally, they will provide education to patients and families about fetal surgery, the potential benefits and potential risks with proceeding with this surgery.

Job Responsibilities:

Performs complete health history, physical examination and psychosocial assessment of pre- and post- operative fetal surgery patients in the outpatient clinic.

Partners closely with the Director of the Fetal Surgery Program to support all aspects of the fetal surgery program.

In collaboration with physicians, orders and coordinates laboratory, radiological and diagnostic studies for routine follow up, and as a response to problems identified in the outpatient clinic.

Reviews testing, differentiates between normal/abnormal findings and follows standard practice for abnormal findings, which may include further diagnostic testing or referral to consulting physicians per customary pattern of the collaborating physicians.

Maintains contact with assigned patient caseload between visits, based on their particular needs. Initiates appropriate responses to issues and problems identified during these contacts.

Coordinating the patients initial referral, pre-consult, initial consultation and post-delivery care.

Interface with multiple providers to ensure optimal pre and post-fetal surgery care.

Act as a partner to all the members of the multidisciplinary team that provides 24-hour on call coverage of patient-related phone calls.

Build and cultivate strong relationships with referring physicians and members of the multi-disciplinary care team that will include maternal fetal medicine specialists, obstetricians, fetal surgeons, pediatric surgeons, social workers, radiologists, cardiologists and genetic counselors

Formulates a plan of care, along with the attending physician, consulting physicians, patient, family and other members of the interdisciplinary team for both preoperative and postoperative fetal surgery patients, based on expected goals of care and length of stay, care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes.

Works closely with insurance case managers and patients to facilitate approval for fetal surgery and needed services and medications.

Coordinates discharge planning with attending physicians, obstetricians, maternal fetal medicine physicians, inpatient nurse practitioners, social work and patient and family, to assure timely and safe discharge. Orders / prescribes discharge medications according to New York State law, including controlled substances with DEA authorization.

Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Educates patient and family to facilitate and promote healthy adjustment to the fetal surgery process for both the mother and the baby.

Acts as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care. Considers needs and behaviors of specific patient age and cultural groups in all patient care.

Supports the mission, philosophy, standards, goals and objectives of the institution, department and service line. Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members.

Participates in the development and revision of interdisciplinary and nursing standards, patient and family education materials as relevant to area of practice and expertise. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, house staff, other fetal surgery team members, other disciplines and ancillary services. Works collaboratively with nursing leadership in the NICU and elsewhere, to ensure timely communications with the nursing staff.

Provides consultation about the fetal surgery process to the nursing staff responsible for the patients care in the hospital setting. Facilitates the continuity of care across the continuum of services, e.g. other facilities, appointments with MDs etc.

Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities.

Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Functions as a respectful member of the health care team.

Promotes own professional growth and development in clinical and managerial role.

Serves as a clinical expert and resource for the education of peers, staff members in the units that take care of fetal surgery mothers and children.

Participate in educational and outreach programs for fetal surgery patients and their families, and other health professionals to increase knowledge about fetal surgery.

Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement.

Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service.

Supports the Director of Fetal Surgery and their collaborating physicians in research efforts related to fetal surgery.

Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise.

Assists in the preparation, implementation, and evaluation of research protocols when applicable.

Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for the service.

Minimum Qualifications:
To qualify you must have a Current Registered Professional Nurse in New York. Baccalaureate degree in nursing required. A minimum of four years of clinical practice in the care of the patient population served; Ability to apply nursing process toward achievement of specific outcomes in an organized fashion which addresses patient/family satisfaction; Demonstrates the potential for leadership; demonstrates ability to develop, guide, motivate, nurture, and coach others; participates in service or departmental committees, staff and/or patient education programs. Evidence of strong interpersonal skills, effective communication skills, and creative problem solving; Membership in pertinent professional organizations; Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities..

Required Licenses: Registered Nurse License-NYS

Preferred Qualifications:
Membership in NATCO, ITNS desirable

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $140,400.00 - $159,270.30 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
