    Advanced Care Provider– ED/Inpatient Navigator
    General Job Summary: Serves as a clinical liaison across care settings to support patient transitions from emergency and inpatient care to outpatient follow-up. Ensures timely coordination, reduces care gaps, and improves continuity of care and patient outcomes.Primary Job ResponsibilitiesACP Core Responsibilities:
    • Reviews ED and inpatient census to identify patients requiring follow-up
    • Management of acute health concerns as needed, including coordination with acute care providers and care team
    • Coordinates post-discharge care, including scheduling and care plans
    • Communicates with physicians, care teams, and patients to ensure continuity
    • Performs clinical outreach and assessment as appropriate
    • Facilitates transitions of care and addresses barriers to access
    • Documents coordination activities in the EHR
    • Supports readmission reduction and care management initiatives
    • Collaborates with practice teams, care management, and external providers
    • Other patient care tasks as directed
    Education and ExperienceRequired:
    • Graduate of accredited Nurse Practitioner or Physician Assistant program
    • Current North Carolina licensure
    • Board certification (NP or PA-C)
    • BLS certification (ACLS preferred where applicable)
    • Strong clinical, communication, and documentation skills
    • Ability to travel between hospital facilities as needed if covering other ACP ED/Inpatient Navigators
    Preferred:
    • Prior experience in any clinical role at the hospital system where the position will be assigned
    • Experience in value-based care, care coordination, case management or population health
    Physical Requirements
    • Work consistently requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
    • Must be able to lift and support weight of 35 pounds.
    • Ability to concentrate on details.
    • Use of computers for long periods of time.
