We emphasize a holistic, “inside-out” approach to wellness, chronic disease management, and preventive care.
Position Overview We are seeking a motivated and experienced
Geriatric Nurse Practitioner (ARNP) to provide on-site medical care to patients in long-term care and residential facilities. This is a mobile position ideal for a provider who enjoys autonomy, building relationships, and making a meaningful impact in the elderly population.
Responsibilities
- Provide primary and preventative care to geriatric patients in ALF, ILF, and SNF settings
- Perform comprehensive assessments, routine follow-ups, and acute visits
- Manage chronic conditions (HTN, diabetes, dementia, etc.)
- Coordinate care with facility staff, specialists, and families
- Complete documentation in EMR in a timely manner
- rotating weekend call
- Participate in care planning and interdisciplinary team meetings
- Ensure compliance with all regulatory and clinical guidelines
Qualifications
- Active Florida ARNP license
- Board Certified (ANCC/AANP)
- Medicare credentialed a plus
- Experience in geriatrics, internal medicine, or long-term care preferred
- Strong clinical assessment and diagnostic skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation (travel between facilities required)
