Adult Case Manager

SpringVale Health Centers is seeking an Adult Case Manager to work with a designated caseload of individuals with on-going mental illness to ensure quality of care, coordination of linkage to appropriate community services, skill development, monitoring of symptoms, and to assure that overall needs of the individual are being met. The Case Manager will provide a variety of functions to aid and support improved self-sufficiency and independent functioning within the community while serving as an advocate for the client. This is a full time, 40 hours per week, non-exempt position.

Qualifications:

Bachelor's degree in Social Work, Psychology, or related field OR 3 years of mental health experience. Must have and maintain an excellent driving record and vehicle for transporting clients in the community. Candidates must have strong communication skills, excellent time management, and display professional decision-making capabilities.
