Position Summary
  • Full-time position for Mental Health Case Manager to assist adult clients living with severe mental illness in Cincinnati/Hamilton County.  Position includes working with clients who participate in specialized programs such as LGBTQ+ and older adult populations.
  • Provide community-based case management services such as but not limited to, symptom monitoring, treatment/service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral/advocacy to community support systems and support of client’s self-determination.
  • Work in a supportive team environment as well as collaboration with a multi-disciplinary clinical treatment team.
  • Meet service delivery expectations.
 
What We Value in our next ADULT CASE MANAGER
  • High School Diploma or Equivalent, bachelor’s degree is Preferred.
  • Case Management experience is not required.
  • Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability.
  • Strong interpersonal skills and ability to provide support in person and by telehealth.
  • Must be able to lift/push/pull up to 20 pounds to support demands within office environment.
