Active Home Health, Hospice & Personal Care is looking for a Full Time RN Admit Nurse. We are looking for someone who is caring, professional and self-motivated to join our team. Job Requirements:
- Initial Visit to assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
- Chart all clinical documentation in our EHR software
- Create a patient plan of care & Coordinate services
- Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
- Provides medication education and management per physician order
- Works hand-in-hand with the IDT and IDG to ensure patient goals are met and appropriate outcomes are achieved
- Shares on-call duties
- Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams
Job Qualifications:
- Registered nurse with current license to practice professional nursing in the state of Utah
- Polite and compassionate
- Excellent observation, problem solving, verbal and written communications
- Maintains a current CPR certification
- Must be a licensed driver with an automobile that is insured, registered and in good working order
- Good communication and smartphone skills
- Ability to pass a background check
