This is a full time position working day shift at Northwest Transitions. Our free-standing Inpatient Rehab facility is located at 2025 W. Orange Grove Rd. Tucson, AZ 85741.
Job Summary
The Admissions Liaison serves as part of the overall admission team and works collaboratively across multiple departments. Works with case management, physicians, therapists and marketing. Confirms that all insurance benefits coverage meets standards of admission as dictated by policy.
Essential Functions
- Conducts comprehensive assessments of patients upon arrival, gathering medical history, current condition, and other relevant information to determine the appropriate level of care.
- Collaborates with physicians, care managers, and other healthcare staff to ensure accurate and timely patient admissions, including necessary documentation and care planning.
- Verifies patient information, including demographics, insurance, and medical records, to ensure compliance with facility admission protocols.
- Communicates effectively with patients and their families, providing information about the admission process, care expectations, and patient rights.
- Supports the coordination of care by initiating orders, treatments, and diagnostic tests as appropriate, based on the admission protocol.
- Maintains accurate and complete documentation of all admission activities in the electronic medical record (EMR), adhering to facility standards and regulations.
- Participates in quality improvement initiatives related to the admission process, identifying opportunities for enhancement and contributing to implementation efforts.
- Provides patient education on facility services, treatment plans, and post-admission care to ensure patient understanding and satisfaction.
- Ensures compliance with all regulatory requirements, including privacy, safety, and infection control, during the admission process.
- Performs other duties as assigned.
- Complies with all policies and standards.
Qualifications
- 1-3 years of nursing experience in an acute care or admissions setting required
Knowledge, Skills and Abilities
- Strong clinical assessment and decision-making skills to facilitate appropriate patient placement.
- Excellent communication and interpersonal skills for effective interaction with patients, families, and healthcare teams.
- Proficiency in using electronic medical record (EMR) systems for accurate patient documentation.
- Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- OR Occupational or Physical Therapy license
- BCLS - Basic Life Support required
