Description

Summary:

  • Schedule: Mon-Fri 1030-1900
  • Location: 1380 Howard Street, San Francisco
  • Work Mode: Onsite
  • Duration: 8 weeks with extension a strong possibility

Responsibilities:

  • Provide non-clinical support, guidance, and assistance for patients and families navigating complex healthcare environments.
  • Act as a communication liaison to understand patients' non-clinical needs, desires, and concerns.
  • Guide patients and families to a broad range of services, amenities, and information to promote healing and ensure satisfaction with the patient care experience.
  • Engage and outreach to patients with a focus on low-threshold access to medications and harm reduction.
  • Conduct outreach to patients referred to OBIC, facilitate appointment barriers, and serve as a resource and coach for clients.
  • Assist with access to other services such as health insurance, primary care, mental health services, social services, and residential treatment facilities.
  • Work closely with the OBIC Substance Use Counselor to serve as a resource for patients, develop treatment plans, and assist with documentation.

Requirements:

  • Experience with underserved communities suffering from substance use addiction.
  • Experience with patient/client scheduling and booking appointments in Epic Hyperspace.
  • Front-desk administrative experience is ideal.

  • Mission-driven, team player committed to patient-centered and compassionate care for people using drugs.
  • Utilize lived experience with substance use and/or living in the communities served to support patient engagement in care.
  • Preference for applicants with connections to, and reflecting the diversity of, the local community.
