Job Overview
Fairview is seeking a dedicated Kidney Transplant Social Worker to join our team in a full-time, benefit-eligible role. We believe the hours for this roll will be 7:30 a.m. to 4:00 p.m. and includes participation in an every-sixth-weekend rotation as well as one holiday per calendar year.
In this role, you will provide comprehensive psychosocial support to kidney transplant recipients throughout their transplant journey, working across both outpatient and inpatient settings. The position is based at The Clinics and Surgery Center and Fairview - University of Minnesota Medical Center, allowing you to collaborate closely with a multidisciplinary transplant team while supporting patients across the continuum of care before, during, and after transplantation.
The acute social worker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute social worker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The social worker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
