    Nurse Practitioner / Physician Assistant Comprehensive Visits (Clinic + Telehealth)

    A growing, innovative healthcare organization is seeking a Nurse Practitioner or Physician Assistant to join its clinical team. In this role, you ll help deliver high-quality care to senior patients by conducting comprehensive visits in clinic and through approved telehealth platforms, with a focus on closing care gaps and supporting preventive and chronic care needs.

    What You ll Do
    • Conduct pre-visit chart reviews to identify clinical and quality opportunities.
    • See scheduled patients in clinic and/or via telehealth to address outstanding preventive and chronic care needs.
    • Perform and interpret evidence-based screenings and point-of-care tests (where available).
    • Maintain accurate problem lists and document clear assessments and care plans in the electronic medical record.
    • Coordinate follow-up care and communicate with the broader care team to support continuity.
    • Provide patient education on disease management, preventive screenings, and medication adherence.
    • Complete documentation within expected timelines and support a high-quality patient experience.
    What We re Looking For
    • Active NP or PA license (or ability to obtain by start date), in good standing.
    • Active national certification (ANCC/AANP for NP; applicable national certification for PA).
    • 1+ year of clinical experience (post-training).
    • Preferred background in geriatrics, internal medicine, cardiology, and/or endocrinology.
    • Comfort working in an EMR environment.
    • Familiarity with Medicare Advantage, risk adjustment (HCC), and quality measures (HEDIS/Stars) is a plus.
    • Ability to meet role-related physical requirements and travel within the licensed state if needed.
