Help People Build Independence in the Community

Pyramid Healthcare is seeking an experienced ACTT Case Manager – Vocational & Housing to join our Assertive Community Treatment Team (ACTT) serving the Mars Hill, NC area.

Qualified Professional (QP) status is required.

ACTT is a “hospital without walls,” providing intensive, community-based behavioral health services to individuals living with severe and persistent mental illness (SPMI). Rather than providing care exclusively in an office or facility, our multidisciplinary team meets individuals where they are and helps them build stability, independence, and meaningful connections within their communities.

In this role, you'll provide hands-on case management and community-based support with a particular focus on housing stability, employment and vocational development, life skills, community resources, and independent living.

What You'll Do

Community-Based Case Management

  • Provide intensive, community-based case management and support to individuals with serious mental illness.

  • Develop and monitor person-centered service and treatment plans based on each individual's needs and goals.

  • Help clients identify strengths, overcome barriers, and work toward greater independence.

  • Connect individuals with community resources, benefits, healthcare, transportation, and other support services.

  • Build trusting, professional relationships with clients while maintaining appropriate boundaries.

Vocational & Employment Support

  • Help individuals identify and pursue employment, education, and vocational goals.

  • Assist clients with job readiness, employment resources, and connections to community employers and services.

  • Support individuals in developing skills needed to obtain and maintain employment.

  • Coordinate with vocational rehabilitation agencies, employers, and other community partners as appropriate.

Housing & Independent Living Support

  • Help clients identify and access housing resources and community-based supports.

  • Assist individuals with maintaining housing stability and navigating housing-related barriers.

  • Connect clients with benefits and resources that support long-term independence.

  • Help individuals develop practical life skills and strategies for successful community living.

Treatment Team & Care Coordination

  • Collaborate with the ACTT multidisciplinary team to coordinate services and monitor client progress.

  • Participate in regularly scheduled treatment team meetings, case reviews, and discharge planning.

  • Communicate with families, referral sources, healthcare providers, employers, housing providers, and community partners as appropriate.

  • Support continuity of care and successful transitions between services.

Documentation & Quality

  • Complete timely and accurate documentation in the electronic health record.

  • Maintain accurate records of client contacts, services, referrals, and care coordination activities.

  • Complete treatment plan reviews and other required clinical documentation.

  • Participate in program quality improvement activities and required training.

Crisis & Community Response

  • Participate in crisis response and rotating on-call responsibilities.

  • Assist with crisis planning and interventions as needed.

  • Provide transportation to appointments, community resources, and other services when required.

  • Respond appropriately to changing client needs while maintaining safety and professional standards.

What We're Looking For

Required

  • Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field.

  • Must meet Qualified Professional (QP) requirements under 10A NCAC 27G.0104.

  • Valid driver's license and reliable transportation.

  • Ability to provide services in homes, community settings, and other non-traditional environments.

Preferred Experience

  • Experience working with individuals with severe and persistent mental illness (SPMI).

  • Experience in case management or community-based behavioral health.

  • Experience with vocational rehabilitation or employment services.

  • Experience with housing services or supportive housing.

  • Knowledge of community resources, benefits, and support services.

  • Experience working as part of a multidisciplinary behavioral health team.

  • ACTT or other intensive community-based behavioral health experience.
