Job Summary and Qualifications We are currently looking for an ambitious Accredited Stroke Program Coordinator to join our team. As Stroke Coordinator, you will be supporting and promoting optimal care for our patients with strokes. In addition, you will facilitate the development, education of nurses, care givers at all levels and family. You will implement and adhere to the Stroke Clinical Practice Guidelines. You will encourage the protocols and therapies to become the standard of care for the facility. If you are dedicated to caring for the well-being of others, this could be your next opportunity. What you will do in this role: You will develop your knowledge and competency related to care of the stroke patient (including clinical practice guidelines, HCA metrics and data, American Heart Association/American Stroke Association information/initiatives, National Institutes of Health Stroke Scale and other applicable guidelines/initiatives).You will demonstrate leadership to promote teamwork, work with multidisciplinary groups and facilitate problem-solving tasks, using critical thinking techniques.You will collect, review and analyze, and presents stroke outcome data to various committees and facility leadership, as well as reports to the division quality leadership. You will be able to articulate the PDSA cycle and apply to processes related to care of the stroke patient within the facility. Create action plans with measurable goals for any outliers or opportunities for improvement.You will work collaboratively with the Vice President of Quality and Patient Safety to complete West Florida Division and Corporate reporting requirements. You will work closely with providers, coding, and Clinical Documentation Improvement Specialists to capture accurate documentation that reflects the patient's condition.You will coordinate the Disease Specific Certification related to Stroke; maintains program according to Joint Commission/DNV standards and elements of performance, manages data related to reporting for the certification and manages recommendations for improvement as needed. Requirements: Current Florida RN licenseRN, BSN Preferred3-5 years clinical experience. Critical Care or ER Experience preferred. Familiarity with The Joint Commission, Medicare, and State requirements.Extensive computer knowledge and experience required with emphasis on EMR, Excel, Word, PowerPoint, and HPF applications.BLS must be obtained within 30 days of hireNIH Stroke Scale must be obtained within 30 days of hire.
