The Role- Provide primary care support for patients with chronic kidney disease
- Manage high-risk patients beyond basic wellness visits
- Play a key role in transitions of care and reducing readmissions
- Collaborate closely with dialysis and interdisciplinary teams
- Build meaningful, ongoing patient relationships
Ideal Candidate- Active California Nurse Practitioner license and Board Certification
- Experience in primary care, internal medicine, or complex care settings
- Comfortable managing high-risk or medically complex patients
- Interest in value-based care and outcomes-driven medicine
- Strong communicator who thrives in a team environment
