    Description

    Tennessee Quality Care Hospice is seeking a licensed PRN Social Worker to join their growing team in Columbia! This role provides counseling, resources, and coordination with our clinical team to ensure holistic care for both patients and families in the comfort of their homes.

    MSW and TN state license required.

    Schedule: PRN. Shifts will likely be Monday-Friday 8:30am-4:30pm

    Territory: Columbia and surrounding counties.

    Qualifications:

    • Social Work Degree

    • Tennessee: Master's Degree in Social Work required.

    • Current State Licensure in Social Work

    • Tennessee: LMSW or LCSW required.

    • Minimum of one year's experience in health care or hospice setting

    • Valid driver's license and proof of insurance is required

    What You'll Do:

    • Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
    • Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
    • Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
    • Provides accurate and timely documentation of patient services to reflect the plan of care
    • Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
    • Serves as liaison between patients and families/caregivers and community agencies.
