    Now Hiring: Full-Time Registered Nurse (RN) - Home Health

    Clarksville, TN

    Schedule: Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

    Key Responsibilities

    • Perform patient assessments, admissions, and ongoing nursing evaluations

    • Develop and update individualized plans of care

    • Coordinate care with physicians and the interdisciplinary healthcare team

    • Provide skilled therapeutic, preventative, and rehabilitative nursing care

    • Monitor patient progress and communicate changes in condition

    • Educate patients and caregivers on disease management, medications, and treatment plans

    • Maintain accurate and timely clinical documentation

    • Deliver compassionate, patient-centered care in the home setting

    Qualifications

    • Graduate of an accredited nursing program

    • Current Registered Nurse (RN) license in the state of Tennessee

    • Minimum of one (1) year of clinical nursing experience (ICU, Med-Surg, or Rehabilitation preferred)

    • Home Health experience preferred, but not required

    • Current CPR certification

    • Valid driver's license, reliable transportation, and proof of automobile insurance
