    What You'll Do

    • Provide skilled physical therapy services in patients' homes following the physician's plan of care.
    • Evaluate patient mobility, strength, balance, and functional abilities.
    • Develop individualized treatment plans to help patients reach their rehabilitation goals.
    • Educate patients and caregivers on exercises, mobility techniques, and safety strategies.
    • Recommend adaptive equipment and home modifications to improve safety and independence.
    • Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient outcomes.
    • Complete accurate and timely clinical documentation.
    • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policies.
    • Monitor patient progress and adjust treatment plans as needed.

    Qualifications

    • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
    • Current Physical Therapist (PT) license in the state of Tennessee.
    • Minimum of two (2) years of experience in acute care or a similar clinical setting preferred.
    • Current CPR and First Aid certification.
    • Valid driver's license, reliable transportation, and current automobile insurance.
