What You'll Do
- Provide skilled physical therapy services in patients' homes following the physician's plan of care.
- Evaluate patient mobility, strength, balance, and functional abilities.
- Develop individualized treatment plans to help patients reach their rehabilitation goals.
- Educate patients and caregivers on exercises, mobility techniques, and safety strategies.
- Recommend adaptive equipment and home modifications to improve safety and independence.
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient outcomes.
- Complete accurate and timely clinical documentation.
- Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policies.
- Monitor patient progress and adjust treatment plans as needed.
Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
- Current Physical Therapist (PT) license in the state of Tennessee.
- Minimum of two (2) years of experience in acute care or a similar clinical setting preferred.
- Current CPR and First Aid certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
