    What You'll Do

    As a Home Health LPN, you'll provide skilled nursing care directly in patients' homes while working closely with the Registered Nurse and interdisciplinary care team.

    Responsibilities include:

    • Providing direct patient care according to the established plan of care
    • Completing skilled nursing visits in patients' homes
    • Administering treatments and medications as ordered
    • Monitoring patients and reporting changes in condition to the RN
    • Educating patients and families on medications, treatments, and self-care
    • Completing accurate and timely clinical documentation
    • Following infection-control and safety procedures
    • Collaborating with the RN, physicians, therapists, and other members of the care team
    • Supporting patients in managing chronic conditions and maintaining independence at home

    Qualifications

    • Current LPN/LVN license in Tennessee
    • Graduate of an accredited practical or vocational nursing program
    • Current CPR certification
    • Reliable transportation
    • Valid driver's license and current automobile insurance
    • 3+ years of nursing experience preferred
    • Home Health, Medical-Surgical, Community Health, or similar nursing experience preferred
