Ensure treatment team members receive pertinent information regarding case identification, linkage and coordination of services for children and families with CBH, other service providers and schools, within CCTC. Ensure the completion of necessary documentation, outcome measures and signatures are obtained on relevant treatment documents and that required documentation is updated within required timeframe.

At Children's Crisis Treatment Center (CCTC), our staff is dedicated to addressing the impact of childhood traumatic events and other challenges. The Care Coordinator will support our growing ABA program and follow through with treatment team recommendations in a faced paced environment and meet the needs of the children ages 2-14 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.

Summary of Essential Functions:

• Develop and implementation of service coordination plan in conjunction with treatment team
• Ensure treatment team members receive pertinent information regarding case identification, linkage and coordination of services for children and families with CBH, other service providers and schools, within CCTC
• Facilitate and attend interagency/IEP meetings as needed; monitor effectiveness of services and reassess service needs if necessary
• Coordinate scheduling to ensure continued supports are in place and request necessary documentation for upcoming appointments
• Ensure that service is being received as prescribed
• Ensure maintenance of timely and accurate electronic chart records
• Ensure the completion of necessary documentation, outcome measures and signatures are obtained on relevant treatment documents and that required documentation is updated within required timeframe
• Document contact and coordination provided
• Provide transportation as needed
• Participate in internal as well as external trainings, ensuring the completion of CCTC mandatory trainings

A minimum of a Bachelor's degree in Social Work, Social Science and Bachelor of Arts degree from an accredited educational institution. Ability to provide direct services to children and adolescents with serious emotional disturbances and their families. Experience in community families with multiple problems.
