Nemours is seeking a Clinical Social Worker (Outpatient Neurology), I FULL-TIME, to join our team in Orlando, Florida.The Social Worker is responsible for facilitating the adjustment of the patient and family to illness and to help the family and the individual function optimally. The social worker will provide psychotherapy and psychosocial support for children with a broad array of diagnoses, including but not limited to adjustment to medical illness, ADHD, depression, anxiety and autism spectrum disorders. Services are provided to assist the family in the best utilization of the healthcare system and link them with the variety of services necessary to meet their individual needs. The social worker must be knowledgeable of age specific principle of growth and development. The social worker must possess effective communication skills needed to collaborate with all members of the health care team. Job Duties: <ul><li>Manage complex assessments of the patient/family's biopsychosocial situation and establishes goals. </li><li>Periodically reassesses the patient/family situation and modify plan as needed. </li><li>Provide services to address social needs of patients receiving care in their assigned clinic, including but not limited to, domestic violence, psychiatric emergency cases, child abuse, family disputes, end of life and bereavement, Social Determinants of Health, and compliance with treatment. </li><li>Participate in the interdisciplinary assessment and treatment of patients and their families and provide consultation for physicians and other members of the healthcare team to determine current psychosocial needs. </li><li>Work with multidisciplinary teams to address the mental and behavioral health needs of patients. </li><li>Act as a liaison between hospitals and the various community agencies, which provides aid to families and children. </li><li>Coordinate and provide therapy sessions for patients showing a need for counseling/therapy. Provide counseling and/or therapy for patients and their families assisting them with emotional, personal, psychological, socioeconomic factors, and adjustment to illness issues, to include the entire spectrum from diagnosis throughout the treatment course. </li><li>Document interventions in the Electronic Medical Record following departmental guidelines. </li><li>Follow Medicaid, CMS and other regulatory/compliance agency requirements for documentation and billing. </li><li>Interface with insurance providers, case managers and others involved in the authorization of needed patient services. </li><li>Establish and maintain effective working relationships across disciplines. </li><li>Provide families with specific community resource information. </li><li>Other duties as assigned. </li></ul> Job Requirements <ul><li>Masters Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).</li><li>Minimum three (3) years of experience required. The Social Worker - MSW will provide comprehensive psychosocial services to patients admitted to Nemours Children's Hospital. The primary focus will be addressing patients' psychosocial needs and enhancing their overall well-being. Additionally, you'll play a crucial role in facilitating patient and family adjustment to illness, ensuring optimal functioning for both individuals and families. The responsibilities encompass assessment, intervention, coordination, and collaboration with interdisciplinary teams. Here are the key aspects of your role:Complex Assessment and Goal Setting: <ul><li>Conduct comprehensive assessments of patients and their families' biopsychosocial situations.</li><li>Establish clear goals and periodically re-assess the patient/family situation.</li><li>Document thorough psychosocial assessments and histories for patients and families.</li></ul> Crisis Intervention Services: <ul><li>Provide timely crisis intervention services, including cases involving domestic violence, psychiatric emergencies, child abuse, and family disputes.</li></ul> Referral Triage and Treatment: <ul><li>Assess patient mental health needs and refer for counseling and psychiatry as needed (both internally and to external community providers)</li></ul> Comprehensive Care Management: <ul><li>Perform admission screenings and psychosocial assessments.</li><li>Offer case management, medical crisis counseling, patient/family education, advocacy, residential placement, community referral facilitation, crisis intervention, and mental health evaluation.</li></ul> Child Abuse Investigations and Domestic Violence Management: <ul><li>Assist with the assessment and coordination of child abuse investigations within the hospital.</li><li>In collaboration with the medical team, manage domestic violence situations.</li></ul>Professional Development: <ul><li>Continuously update your education and skill level in the field.</li><li>Attend in-service presentations and continuing education programs to maintain and enhance your knowledge base.</li></ul> Hospital Discharge Planning: <ul><li>Asses social barriers to discharge, work collaboratively with case management, participate in rounds, and escalate complex cases to avoid discharge delays.</li></ul> On-Call Responsiveness: <ul><li>If on-call, promptly respond to events and service requests within 5 minutes of receiving a phone call.</li><li>Remain within a 30-minute drive of the hospital.</li></ul> Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisorPosition requirements <ul><li>Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) is required.</li><li>MSW from an institution accredited by the Council of Social Work Education (CSWE) is required</li><li>Minimum one year of social work experience is required; 3+ years is preferred (in lieu of one year of experience, graduate school internship may be applied).</li></ul>About Us Nemours Children's Health is an internationally recognized pediatric health system serving more than 1.7 million patient encounters each year.
