Summary

Youth Program Coordinator II supports the Carson Tahoe Vehicles of Change (CTVs) mobile preventive mental health program to strengthen youth mental health and well-being by helping young people build the skills, resilience, and connections needed to thrive across the Quad Counties (Carson City, Douglas, Lyon, and Storey) in Northern Nevada. This role is embedded exclusively within CTVs and focuses on sustained engagement with youth in schools, community organizations, and partner sites.

This position serves as a trusted connector between children, families, community partners, and Carson Tahoe Health resources. Responsibilities include facilitating youth programming, supporting sensitive conversations and data collection, identifying strengths and needs, and connecting participants to appropriate health system and community resources. This position works closely with the Carson Tahoe Community Wellness team to ensure programming is coordinated, responsive, and aligned with community needs.

This role is compassionate, culturally responsive, skilled in working with children and families, and comfortable supporting sensitive, trauma-informed work while maintaining appropriate boundaries and confidentiality. Must be willing to travel throughout the Quad Counties and work a flexible schedule.

Qualifications

Required:

  • One (1) of the following:

  • Three (3) years experience working with youth (education, after-school, community programs, etc.)

  • Current Nevada CHW I certification with two (2) years experience supporting youth or families

  • Associate's degree or higher in a related field with one (1) year experience working with children

  • Experience supporting group activities or structured programming for youth

  • Ability to manage behavior, engagement, and transitions in group settings

  • Strong communication with youth and families

  • Valid unrestricted driver's license and willingness to travel throughout the Quad Counties

  • Current American Heart Association (AHA) Basic Life Support (BLS) for healthcare providers within ninety (90) days of hire

Preferred:

  • Bachelor's degree in a related field

  • Community Health Worker II (CHWII) certification

  • Experience in school-based or youth-serving organizations

  • Experience connecting families to community resources

  • Experience with data collection, program evaluation, or electronic documentation systems

  • Bilingual or multilingual proficiency

Essential Functions

  • Facilitate and co-lead resilience-building and wellness programming for children and families through CTVs at schools, Boys & Girls Clubs, and community sites.

  • Build consistent, trusting relationships with youth and caregivers through ongoing engagement rather than one-time interactions.

  • Support trauma-informed discussions related to stress and coping in an age-appropriate manner.

  • Assist with activities and data collection as outlined by program protocols and under appropriate supervision.

  • Identify participant needs and connect children and families to Carson Tahoe Health services and community resources, including primary care, behavioral health, social services, and prevention programs.

  • Provide follow-up support to families to help navigate referrals and reduce barriers to care.

  • Collaborate closely with CTH Community Wellness team to align programming, logistics, and outreach efforts.

  • Maintain accurate documentation of participant engagement, referrals, and outcomes in accordance with program and regulatory requirements.

  • Collect qualitative feedback and observations to support program evaluation and continuous improvement.

  • Participate in program planning meetings, trainings, and debriefs.

  • Represent CTVs and Carson Tahoe Health professionally in community settings.

  • Maintain awareness of local resources and eligibility requirements.

  • Support community events and longer-term programs while maintaining a child-centered focus.

  • Perform other related duties as assigned in support of CTVs.
