Since 1991, Cocoon House has been committed and working towards breaking the cycle of homelessness for teens, young people and their families throughout Snohomish County. The Youth Housing Case Manager , internally referred to as the Transitions Case Manager is responsible for creating and implementing case plans for the youth in Transitional Living Program for 16 and 17 year olds. As a key member of the Youth Housing team, the Case Manager works to ensure that youth in the Transitional Living Program are setting and maintaining progress toward goals in various domains including basic needs, life skills, education, employment, health, mental health, and substance use. You will be responsible for developing individualized plans to meet the needs of each youth, as well as providing guidance, advocacy, and professional support. The Case Manager should have a good understanding of the unique challenges facing youth in our community and be able to build strong relationships with them to ensure their success while maintaining healthy boundaries. Youth Housing Case Manager Duties & Responsibilities Full-Time | On-site in Everett Starting Wages: $23-25/hr DOE| $1.50/hr pay differential for multi-lingual skills with additional paperwork Must have weekend & evening availability Maintain a high level of supervision of youth in care, ensure safety at all times and help ensure youth are following program guidelines and expectations. Develop and manage individualized case plans for youth residents in the Transitional Living Program through ongoing meetings and check ins Facilitate and coordinate communication between clients, family members, providers, and other agencies Make appropriate referrals for medical, dental, health, and substance use needs De-escalate and provide crisis intervention with clients Assist clients in accessing services and resources Maintain accurate and up-to-date client records Follow mandated reporter requirements through documentation of incidents and calling CPS appropriately Ensure that clients are following guidelines of the program Collaborate with fellow staff and partners regarding youth needs and case plan follow through Attend community meetings and direct wrap around meeting with staff and youth Transport youth to appointments as needed Participate in on-call rotation Benefits: Medical, Dental, and Vision health options with employer contribution* 403(b) retirement plan with agency match* 21 days PTO accrual in first year* 10 agency-recognized holidays Multilingual pay differential opportunities Paid training & testing: CPR/first aid, food handler's permit, blood borne pathogens, abuse awareness, TB testing, and additional in-depth practical trainings Compensation details: 23-25 Hourly Wage PI372ad8438ca5-25448-408446225c143e31-5e48-4549-b638-05792d185386
