Position: Intensive Care Coordination with ICCW (ICCW) Facilitator (Wraparound Facilitator) Status: Full Time, Part-Time/Contingent (Non-Exempt) Reports to: ICCW Supervisor Job Summary: Intensive Care Coordination with Wraparound (ICCW) provides service and support, information and referrals for your family based on a plan specifically designed to strengthen how family functioning. The plan is created around the individual needs of your family. ICCW works with your family to identify a support team of people you trust that builds on your family’s strengths to help meet your family’s needs. The ICCW Facilitator is a community based position, responsible for facilitating family preservation and support services to families eligible for ICCW services. The ICCW Facilitator will assist the family to identify and help with issues such as safety/crisis, parenting, health/medical, emotional, social, housing, education, transportation and legal. Families eligible for this program have a 7- 16 years old with a Serious Emotional Disturbance, intellectual/development disability or serious mental illness. Services will be provided in the office, the clients’ homes, school or other appropriate community outlets. Primary Duties and Responsibilities
