Job Title: Wound Care Nurse Practitioner (AGNP/CNP) | On-Site Senior Living Care Model.

Company: Twin Cities Physicians.

Compensation: $105,000 to $120,000 annually + autonomy + impact-driven care.

Position Summary:
Join a model built for providers who want more than clinic-based care. As a Wound Care focused Nurse Practitioner, you will deliver hands-on, high-impact care directly inside senior communities, managing complex wounds while serving as a trusted clinical partner to staff and families. This role offers true autonomy, consistent patient relationships, and the ability to see the direct results of your care over time. You will be part of a team that values clinical excellence, responsiveness, and innovation in geriatric medicine. Wound care experience or certification is highly preferred.

Responsibilities:
• Provide on-site rounding care and advanced wound management within senior living communities.
• Perform routine and complex assessments, developing evidence-based treatment plans.
• Support continuity of care by following patients across different care settings as needed.
• Collaborate with community staff, physicians, and interdisciplinary teams to optimize outcomes.
• Deliver proactive care coordination, reducing ER visits and hospitalizations.
• Communicate clearly with families and caregivers, building long-term trust.
• Utilize technology and documentation tools to support efficient, high-quality care delivery.

Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota.
• Experience in geriatrics, primary care, or wound care preferred.
• Valid driver’s license and ability to travel between communities.
• Passion for relationship-based, patient-centered care in senior populations.
• Strong clinical judgment with the ability to work independently.
