Job Details

We are seeking an experienced Wound Care RN to oversee and coordinate wound management services within a skilled nursing and rehabilitation setting. This role will focus on evaluating, treating, documenting, and monitoring patients with wounds, pressure injuries, and skin integrity concerns.

The ideal candidate is clinically strong, detail-oriented, and passionate about delivering high-quality patient care.

Responsibilities:
Perform wound and skin assessments
Develop and implement individualized treatment plans
Monitor healing progress and modify treatments as necessary
Collaborate with physicians and interdisciplinary teams
Maintain accurate EMR documentation
Educate staff, patients, and families on wound prevention and care
Ensure compliance with infection control and regulatory standards
Participate in quality improvement initiatives

Qualifications:
Active RN license in New York
Previous wound care experience preferred
Experience within skilled nursing, rehab, or long-term care strongly preferred
Wound Care Certification (WCC or WOCN) preferred but not required
Strong communication and organizational skills
Ability to work independently while collaborating with clinical teams
