Key Responsibilities:
- Provide foot care and/or wound care to residents in long-term care facilities
- Conduct thorough wound assessments and develop personalized care plans for each patient
- Perform wound debridement, dressing changes, and other wound care procedures as needed
- Continuously monitor and assess patients’ progress in wound healing and foot health
- Educate patients and facility staff on effective wound care techniques, prevention strategies, and foot care best practices
- Travel to designated nursing homes in your area
- Communicate regularly with key facility staff, physicians, and healthcare professionals to ensure seamless care delivery and optimize patient outcomes
- Maintain accurate and up-to-date patient records and documentation
- Manage pain related to wounds to ensure patient comfort
- Maintain flexibility while seeing patients during weekday working hours
Qualifications:
- Master’s or Doctoral degree in Nursing from an accredited program
- Current Nurse Practitioner license in Louisiana
- Certification in Wound, Ostomy, and Continence (WOCN) is preferred but not required
- Minimum of 2 years of experience in wound care and/or foot care
- Strong clinical skills in assessment and treatment of wounds and foot conditions
- Independent, organized, and motivated to provide exceptional patient care
- Excellent communication and interpersonal skills
