The successful candidate will:Develop and manage an individualized wound care treatment plan for optimal healing outcomes or chronic maintenanceDemonstrate knowledge of up-to-date topical wound care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapyProvide hands-on wound care to patients, including debridement of wounds by enzymatic and autolytic methods when appropriateOrder, review, interpret; and integrate diagnostic studies and laboratory findings for initiating or modifying the treatment planCollaborate with the patient primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive planUpdate the primary care provider on the status and management plan of the wound as appropriatePrescribes medications and/or durable medical equipment to assist in healing woundsAssist with the identification of wound care quality measures and provide data and documentation for tracking quality measuresWork with nursing leadership and the Director of Nursing Education to determine opportunities for education of nursing staff with appropriate identification of wound types, prevention of wounds, and nursing care of woundsDevelop wound care education plans in collaboration with nursing leadershipInstruct patient/family regarding wound treatments and management plan for optimal wound outcomesTriage telephone calls and provides consultation and serves as a resource to the frontline nursing staffParticipate in professional development activities and maintains professional affiliationRequired Qualifications:Masters Degree in NursingGraduate of an approved Nurse Practitioner ProgramNew York State Registered Nurse and Nurse Practitioner LicensureCertified by the American Academy of Nurse Practitioners, the American Nurses Credentialing Center or both5+ years of experience in wound careWound, Ostomy, Continence Nurse (WOCN) certificate or ability to become certified within 6-months of employmentBLS and CPR certifiedAbility to establish and maintain effective working relationships with the interdisciplinary teamAbility to function independentlyPreferred Qualifications:Work Schedule:Variable Days; Variable Hours(Full-Time)Salary Grade/Rank:SL-5Salary Range: Commensurate with experience and qualificationsExecutive Order:Pursuant to Executive Order 161, no State entity, as defined by the Executive Order, is permitted to ask, or mandate, in any form, that an applicant for employment provide his or her current compensation, or any prior compensation history, until such time as the applicant is extended a conditional offer of employment with compensation.
