JOB PURPOSE

The WHC Care Coordinator ensures eligible women receive timely access to breast and cervical cancer screening services. This role is responsible for coordinating diagnostic testing, facilitating referrals, and supporting members as they navigate the healthcare system. The Care Coordinator promotes continuity of care by guiding members through screening, diagnosis, and treatment, while advocating on their behalf to address barriers and ensure appropriate, patient-centered support throughout their care journey.

KEY RESPONSIBILITIES AND ACCOUNTABILITIES

  • Administer the Women's Health Connection (WHC) program in accordance with Access to Healthcare Network (AHN) policies, procedures, and program requirements
  • Apply critical thinking and follow ASCCP guidelines to determine appropriate diagnostic services and care pathways
  • Accurately enter and maintain WHC member data in the NSHD CAST system in a timely manner
  • Process provider Health Insurance Claim Forms (HICFs) accurately and within established program performance standards
  • Assist members in overcoming barriers and navigating the healthcare system to access needed services
  • Build and maintain effective working relationships with healthcare providers and WHC members
  • Deliver provider training and ongoing education to ensure understanding and compliance with WHC program guidelines
  • Educate members on WHC program policies, procedures, and available services
  • Maintain high standards of data accuracy and integrity
  • Monitor CDC Minimum Data Elements (MDE) requirements and manage assigned caseloads and pending cases
  • Track and coordinate patient progress through breast and cervical referral pathways using case management standards
  • Respond promptly to incoming calls, voicemails, internal communications, and eFax systems
  • Monitor pending services and ensure completion in alignment with program performance indicators
  • Maintain accurate and complete program documentation and records
  • Follow up on incomplete claims or cases requiring additional information or clarification
  • Utilize ASCCP algorithms to guide appropriate care for WHC participants
  • Collect eligibility and enrollment information in compliance with HIPAA regulations, ensuring confidentiality and professionalism
  • Refer members to appropriate social service resources and programs for cancer treatment and support
  • Facilitate enrollment of eligible women into Nevada Breast & Cervical Medicaid
  • Perform general administrative duties and other responsibilities as assigned

QUALIFICATIONS

Education & Experience

  • Bilingual (Spanish and English) - Required
  • High school diploma or equivalent required; Associate degree in social work, public health, sociology, psychology, or a related field preferred, or an equivalent combination of education and experience
  • CHW Certification required within 90 days of hire
  • Minimum of two (2) years of experience in healthcare-related social work and working with special needs populations, including case management
  • Strong communication skills with the ability to interact professionally with members, providers, and staff
  • Experience working in a clinical or patient care office environment preferred
  • Preferred experience with healthcare systems and patient navigation tools, including CAST, Salesforce, and ShoreTel phone system
  • Proficiency with computer systems, including Microsoft Office applications, data entry, and database management
  • Strong proficiency in data entry and 10-key operations, demonstrating a high degree of accuracy.

WORK ENVIRONMENT

  • The noise level in the work environment is usually moderate, and the employee usually works in a climate-controlled office environment
  • Be able to sit at a computer for up to 6 – 8 hours per day
  • Be able to stand between 3 – 6 hours per day.
  • Be able to withstand hand/wrist deviation and repetition
  • Must be able to lift, carry up to 30 pounds with a fair consistency
