A community health organization is seeking Wellness Coaches to provide case management, wellness support, and community resource coordination for adults in a residential setting. This role is fully onsite and works with a high-needs population in a team-based environment.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday through Friday or Tuesday through Saturday
- Hours: 9:00 AM to 5:00 PM; one late day per week until 6:00 PM may be required
- Setting: Onsite
Responsibilities:
- Provide case management support to an assigned caseload of residents
- Develop, monitor, and update individual support plans
- Conduct wellness checks, tenant visits, and follow-up support
- Assist residents with community referrals, advocacy, and access to services
- Document services, case notes, incidents, and resident interactions accurately
Qualifications:
- Bachelor's degree
- Working knowledge of common computer applications, including email, Microsoft Word, Excel, PowerPoint, and Windows-based systems
- Case Management experience
Desired Skills:
- Experience working with adults with mental health conditions, substance use concerns, or histories of homelessness
- Experience in supportive housing, group work, tenant engagement, community outreach, or referral-based services
- Bilingual (English/Spanish)
- Familiarity with electronic health records, especially AWARDS
