Location: Gap, Pennsylvania Schedule: Full-Time Weekends | Two Shift Options Travel: Local Travel Required Education Requirement: Associate Degree or Higher

The Weekend Hospice RN will manage and deliver individualized patient care plans, support patients in Gap and surrounding areas, and consistently meet critical deadlines for charting and documentation. The ideal candidate is an experienced hospice nurse who excels in autonomy, time management, and patient-centered care.
Provide management and delivery of individualized hospice patient care plans Support patients in Gap, PA and surrounding areas Conduct comprehensive assessments and ongoing evaluations of patient needs Consistently meet critical deadlines for charting and documentation Collaborate with interdisciplinary hospice team members to ensure coordinated care Educate patients and families on care plans, symptom management, and available resources Self-manage time, schedule, and daily patient visits Maintain compliance with all regulatory, clinical, and organizational standards
Full-time weekend schedule with two available shift options: Friday / Saturday / Sunday — 8:00 AM to 8:00 PMSaturday / Sunday / Monday — 8:00 AM to 8:00 PM
Associate Degree or higherValid Registered Nurse (RN) license in Pennsylvania (PA)2+ years of Hospice experience Strong ability to self-manage time, schedule, and documentation Compassionate approach to end-of-life care Excellent communication and patient advocacy skills
