Southeast Healthcare's RREACT program is seeking weekend Outreach Coordinators to work in partnership with local emergency departments to engage directly with patients who have experienced an opioid overdose in the hospital to assess their needs and provide immediate referrals to detoxification services, addiction treatment programs, primary medical care, and other necessary resources.

Key Responsibilities:

  • Engage with patients to assess their needs and desires for treatment and recovery.
  • Coordinate referrals to immediate detox services, addiction treatment programs, primary care providers, and other supportive services.
  • Provide education and emotional support to patients regarding available treatment options.
  • Collaborate with a multidisciplinary team to ensure a holistic approach to care and recovery.
  • Maintain up-to-date knowledge of local resources and treatment options for opioid addiction.
  • Document all interactions and follow-up activities accurately and promptly.
  • Monitor patient progress and continue to offer support and referrals as needed throughout the recovery process.
  • Minimum of AA/BA required.
  • CDCA or higher credential required.
  • Experience working with adults with severe and persistent mental illness preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Crisis intervention and de-escalation skills.
  • Knowledge of community resources and healthcare services.
  • Valid driver's license and insurance required.
