Collaborates and consults with physicians Structural Heart Coordinators nursing care team and other healthcare providers in the coordination of care delivered to the Vascular patient populations. Is an integral member within Heart Vascular and Stroke co-management care networks. Works closely with the Leadership team on identifying opportunities for improvement in reducing care variation across the system at each entity level related to vascular care. Previous clinical navigation or project coordination and management will compliment this position. Experience in leading teams to drive changes within the system through development and patient safety improvement projects. Additionally a minimum of two years experience in the clinical setting of vascular area is also preferred.
