Key Responsibilities
The Triage Coordinator is responsible for the coordination of all after hours calls, after hours orders, and after-hours communication with on-call provider, patients, facilities, and families for Hospice. Responsibilities are completed with limited supervision and after hours. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population.

Weekdays: 2 days per week (Monday-Friday), 11:00 AM - 11:30 PM
Weekends: Every other Saturday and Sunday, 8:00 AM - 8:30 PM (remote)
