Key Responsibilities- Provide direct speech therapy services to home health clients across an assigned service area
- Collaborate with interdisciplinary team members to develop and implement comprehensive patient plans of care
- Conduct evaluations and deliver evidence-based speech, language, and swallowing interventions in the home setting
- Utilize and maintain familiarity with rehabilitation equipment including language and communication aids, devices, and wheelchairs
- Document patient progress and maintain accurate clinical records in accordance with agency standards
Required Qualifications- Minimum of 2 years of SLP home care experience required
- Master's or Doctorate Degree in Speech-Language Pathology required
- Certificate of Clinical Competence (CCC) required; Clinical Fellow candidates accepted
- Active Massachusetts Speech and Language Pathologist License required
- Active American Heart Association BLS certification required
- Valid driver's license and personal vehicle required
- Ability to work independently with minimal supervision
Preferred Experience / Skills- Acute care experience preferred
- Experience with rehabilitation and communication assistive devices
