Key Responsibilities of the Speech Language Pathologist
  • Perform patient assessments to determine suitability for home care and progress toward goals.
  • Collaborate with patients, families, and physicians to develop individualized care plans.
  • Assess patient swallowing, communication, and cognitive abilities as needed.
  • Maintain a safe environment, assessing social risks and adhering to infection control measures.
  • Implement treatment programs, including communication and cognitive training.
  • Provide discharge planning and communicate with healthcare teams as necessary.
  • Comply with home care regulations and document all patient interactions accurately.
  • Engage in interdisciplinary communication and ensure appropriate service utilization.
  • Travel to patient homes and facilities as required to deliver personalized care.
  • Participate in agency business growth by orienting new employees and cross-training.

Requirements of the Speech Language Pathologist
  • Graduate of an accredited speech and language program with a minimum of a Master's degree.
  • Current NJ ST License.
  • Minimum of one (1) year of experience in an acute, subacute, or home care setting.
  • Good customer service and communication skills.
  • Home Health Experience required.
