Job Summary (Speech-Language Pathologist) List Format
- Assess, diagnose, and treat residents with speech, language, cognitive-linguistic, and swallowing disorders in a skilled nursing facility. - Develop and implement individualized treatment plans to promote resident independence, safety, and quality of life. - Conduct comprehensive evaluations and provide direct therapy services for communication, cognition, and swallowing. - Perform and interpret swallow assessments; make recommendations for diet modifications as needed. - Document all evaluations, treatments, progress notes, and discharge summaries according to regulations. - Educate residents, families, and staff on therapy plans, communication strategies, and safe swallowing practices. - Participate in care planning and interdisciplinary team meetings, contributing to resident-centered care goals. - Monitor resident progress and adjust treatment plans as appropriate to achieve optimal outcomes. - Ensure compliance with professional, legal, and regulatory guidelines. - Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs) as required. - Collaborate effectively within a multidisciplinary team in a skilled nursing/long-term care environment.
