Job Summary
The Speech-Language Pathologist (SLP) is responsible for evaluating, diagnosing, and treating residents with speech, language, cognitive-communication, and swallowing disorders in a long-term care setting. The SLP works collaboratively with interdisciplinary teams to improve residents’ functional independence and quality of life.
Key Responsibilities
Evaluate residents for speech, language, voice, fluency, cognitive-communication, and swallowing disorders
Develop and implement individualized treatment plans
Provide therapy services in accordance with physician orders and care plans
Perform bedside swallow evaluations and recommend diet modifications
Document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory standards
Educate residents, families, and staff on communication and swallowing strategies
Collaborate with physicians, nursing staff, occupational therapists, and physical therapists
Participate in care plan meetings and interdisciplinary team discussions
Ensure compliance with Medicare, Medicaid, and state regulations
Qualifications
Master’s degree in Speech-Language Pathology
Active SLP license in the state of practice
Certificate of Clinical Competence (CCC-SLP) from American Speech-Language-Hearing Association preferred
CPR/BLS certification (if required by facility)
Previous experience in skilled nursing facility (SNF) or LTC setting preferred
