About the RoleWe are seeking a consistent and experienced Speech-Language Pathologist (SLP) to provide high-quality therapy services within a sub-acute rehab setting. This position involves working across two nearby nursing home buildings, offering support to diverse adult and geriatric patient populations.
Key ResponsibilitiesEvaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders
Develop individualized treatment plans based on patient needs and goals
Provide therapy services within sub-acute rehab and long-term care environments
Collaborate with interdisciplinary teams, including PT, OT, nursing, and case management
Complete documentation according to facility and regulatory requirements
Support PDPM-aligned therapy planning and reimbursement accuracy
Required Skills & ExperienceSub-acute rehab experience preferred
Strong understanding of PDPM reimbursement or willingness to learn
Excellent communication, collaboration, and patient-education skills
Required Licenses & CertificationsWork EnvironmentNursing home / sub-acute rehab setting
