Responsibilities
- Conduct psychosocial assessments for patients and families, identifying barriers to discharge and coordinating needed services.
- Develop and implement discharge plans, arranging post-acute care, community resources, and appropriate placements.
- Collaborate with multidisciplinary teams (nursing, social work, case management, physicians) and participate in rounds and care conferences.
- Provide crisis intervention and emotional support, facilitate family meetings, and advocate for patient preferences and goals of care.
- Document assessments, plans, and progress in the electronic health record; ensure regulatory compliance and patient confidentiality.
Requirements
- Active Georgia Licensed Clinical Social Worker (LCSW) license
- AHA Basic Life Support (BLS) certification
- Minimum of 2 years of recent medical social work experience in an acute hospital setting
- Strong assessment, communication, and care coordination skills
- Ability to work a weekday 8x5 shift; 40 hours per week; able to work in a hospital environment
- Eligible to work in the Lawrenceville, GA area as a travel assignment
