Responsibilities
- Provide clerical support for Case Management, including prior authorizations and pre-cert reviews
- Manage post-acute referrals and discharge planning coordination
- Deliver Medicaid notices to patients and maintain Medicaid/Medicare familiarity
- Document care coordination and transitions in EPIC to support seamless patient flow
- Coordinate referrals to home health, DME, rehabilitation, and long-term care services
- Collaborate with interdisciplinary teams (PT, Pharmacy, Transportation, Interpretation Services, Social Services) to ensure timely care

Requirements
- EPIC charting experience is required
- Minimum 1 year of related experience; first-timers accepted
- Washington state license not required for this clerical-focused role
- Ability to work in a on-site case-management environment
- Clinical background in acute-care settings (ED, ICU, med-surg, etc.) is helpful but not required
- COVID-19 vaccine not required
- Orientation ~40 hours provided during onboarding
- Scrubs: any color allowed; parking is facility-paid
