Position Overview
The Crisis Triage Specialist (BSW) serves as a frontline behavioral health professional providing crisis intervention, assessment, triage, and support services for individuals experiencing mental health or substance use emergencies. This role combines direct client care, risk assessment, de-escalation, documentation, and coordination of care to ensure clients receive the most appropriate level of treatment and support.
Core Responsibilities
Crisis Assessment & Triage
- Conduct brief mental health and safety assessments utilizing evidence-based screening tools such as the C-SSRS and SAFE-T.
- Evaluate clients presenting with psychiatric, emotional, behavioral, or substance use concerns.
- Determine appropriate disposition, including referral, discharge planning, or escalation to a higher level of care.
- Assist with admissions and transitions to crisis stabilization services or other treatment settings as indicated.
Crisis Intervention
- Provide immediate support and de-escalation for individuals experiencing emotional or behavioral crises.
- Implement therapeutic interventions to reduce risk and promote stabilization.
- Maintain a safe and supportive environment for clients in crisis.
- Offer basic comfort measures and emotional support throughout the assessment process.
Client Support & Therapeutic Engagement
- Monitor client safety and emotional well-being.
- Encourage healthy coping skills, positive social interactions, and engagement in treatment.
- Facilitate psychoeducational, life skills, and therapeutic group activities as assigned.
- Support clients in establishing and maintaining daily structure and routines.
Care Coordination & Referral Management
- Coordinate referrals and collaborate with internal and external care teams.
- Connect clients with community resources, outpatient services, and follow-up care.
- Communicate placement recommendations and treatment needs to appropriate stakeholders.
Documentation & Communication
- Complete timely, accurate, and thorough clinical documentation.
- Record assessments, interventions, observations, and client progress.
- Maintain compliance with organizational, regulatory, and billing requirements.
- Participate in interdisciplinary communication regarding client care and treatment planning.
Additional Duties
- Assist with client orientation, intake processes, and basic logistical needs.
- Support transportation coordination, meal monitoring, and other client care activities as needed.
- Perform related duties to support daily operations and client services.
Preferred Qualifications
- Bachelor's degree in Social Work (BSW) or related behavioral health field.
- Experience in crisis intervention, behavioral health, psychiatric services, social work, case management, or community mental health.
- Knowledge of suicide risk assessment, de-escalation techniques, and crisis response protocols.
- Strong communication, documentation, and critical thinking skills.
- Ability to work effectively in fast-paced, high-acuity environments.
