Job Requirements
  • Education Requirements: Graduate of an accredited nursing program with an RN degree (ADN or BSN).
  • Certifications Requirements:
    • Certifications Requirements: Current CPR certification (BLS).

    • Other Requirements: Ability to work Friday/Saturday/Sunday; schedule may be adjusted within the shift options discussed during the interview.
    • Other Requirements: Strong communication and teamwork skills to coordinate with nursing assistants, rehab therapists, and facility staff.
    • Other Requirements: Ability to manage multiple patient needs safely during 12-hour shifts (6a–6p / 6p–6a).

Job Responsibilities

- Provide comprehensive nursing care to patients in a SNF/Rehab and Long Term Care setting during scheduled weekend shifts (Friday/Saturday/Sunday).

- Execute medication administration, treatment coordination, and monitoring according to physician orders and facility protocols.

- Collaborate with interdisciplinary teams (rehab therapy, nursing assistants, and facility leadership) to support patient outcomes and care plan goals.

- Maintain accurate, timely documentation including nursing notes, vitals, wound/skin assessments, and response to interventions.

- Document and communicate patient condition changes promptly to the charge nurse and/or provider, ensuring continuity of care throughout the shift.
