Job Requirements

  • Education Requirements: Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
  • Certifications Requirements: - Valid, unrestricted Texas RN license or license from a compact state. - Basic Life Support (BLS) certification. - Advanced Cardiovascular Life Support (ACLS) certification (preferred but not mandatory).
  • Experience Requirements: - 1-2 years of RN case management experience. - Experience working with individuals with intellectual and developmental disabilities (highly preferred). - Prior experience in long-term care or supportive living settings.
  • Other Requirements: - Strong communication and interpersonal skills. - Ability to collaborate effectively with interdisciplinary teams. - Proficiency in electronic medical records (EMR) systems.

Job Responsibilities

  • Provide comprehensive nursing care and support to residents with intellectual and developmental disabilities, ensuring individualized treatment plans are followed and adjusted as necessary.
  • Collaborate with interdisciplinary teams, including physicians, families, and support staff, to coordinate services and align treatment goals.
  • Perform detailed clinical assessments and monitor health trends to proactively address potential issues and improve patient outcomes.
  • Maintain accurate and timely documentation of all healthcare interventions, assessments, and care plans within electronic medical records systems.
  • Ensure compliance with federal and state regulations, including ICF-IID standards, and stay updated on relevant policies affecting care delivery.
