We are seeking an experienced RN Case Manager to join a collaborative care management team. This role is essential in coordinating patient care across the continuum; from pre-admission through post-discharge; ensuring optimal clinical outcomes, efficient resource utilization, and a seamless patient experience.
You will serve as a key liaison between patients, families, physicians, and multidisciplinary teams while guiding care planning and discharge processes.
Coordinate and oversee patient care plans across disciplines, ensuring timely and appropriate care
Collaborate with physicians on treatment plans, length of stay, and discharge planning
Monitor diagnostics, treatments, and procedures to align with clinical and coverage guidelines
Act as a liaison between patients, families, care teams, and payors
Focus on complex cases, including frequent ED utilizers, chronic conditions, substance use, and vulnerable populations
Facilitate transfers to tertiary care centers and ensure continuity of care
Support compliance with regulatory and accreditation standards
Analyze trends in frequent admissions and contribute to care improvement strategies
Mandatory:
Active RN license (state of practice)
Minimum 1 year of acute care case management experience
Preferred:
Bachelor of Science in Nursing (BSN)
Case Management Certification (CCM or equivalent)
Prior experience in a care management role
