Job Requirements

Education

• Graduate of an accredited Physical Therapy program.

Certifications & Licensure

• Active Physical Therapist license in the state of Oregon (or compact eligibility if applicable).
• Current CPR certification.
• Basic Life Support (BLS) certification required.

Experience

• Minimum of 1 year of recent experience as a Physical Therapist in a Skilled Nursing Facility (SNF) setting required.
• Previous experience with Net Health EMR/documentation system required.

Additional Qualifications

• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Must be able to pass all required background screenings and onboarding requirements.
