• Evaluations & Assessments: Conduct patient assessments to evaluate the level of function by applying diagnostic and prognostic functional ability tests. Evaluate the status of all body systems as required for CMS documentation.
• Care Plan Development: Consult with the physician in the development of the therapy plan of care to address functional limitations and occupational therapy needs.
• Skilled Interventions: Provide services within the scope of practice defined by state laws and in accordance with the established plan of care, utilizing evidence-based techniques.
• Monitoring & Reporting: Observe, record, and report the patient's response to treatment and any changes in the patient's condition to the nurse supervisor and the physician.
• Home Assessments: Evaluate the home environment for safety risks, need for adaptive equipment, and support for daily living activities.
• Care Coordination: Collaborate with other members of the healthcare team to ensure a seamless and patient-centered continuum of care.

• Licensure: Current and active Occupational Therapist (OT) license.
• Experience: Minimum of 1 year of recent home health care experience.
• Technical Skills: Proficiency and hands-on experience using the Homecare Homebase (HCHB) EMR system.
