Physical Therapy Assistant (PTA) – Home Health

Schedule / Job TypeContract | 13 Weeks | Full-Time | Monday through Friday, 8:00 AM – 5:00 PM | No on-call or weekend requirements
Key Responsibilities- Provide physical therapy assistant services to home health patients within an assigned territory
- Conduct follow-up visits and Start of Care visits in accordance with daily productivity expectations
- Meet a daily productivity standard of 6 points per day, which may include a combination of follow-up and Start of Care visits
- Travel to patient homes within the designated service territory
- Document patient care accurately and in a timely manner using the required electronic health record system
Required Qualifications- Active Physical Therapist Assistant (PTA) license in the state of California
- Experience with HCHB (Homecare Homebase) electronic medical records system required
- Prior home health experience
- Valid driver's license and reliable transportation for patient home visits within the assigned territory
Preferred Experience / Skills- Familiarity with the greater Los Angeles/South Bay service area
- Experience completing Start of Care assessments in a home health setting
