Key Responsibilities

• Deliver physical therapy treatment to residents in a skilled nursing facility setting under the supervision of a licensed Physical Therapist
• Implement individualized treatment plans and document patient progress accurately and in a timely manner
• Assist in the assessment and modification of therapy programs to meet the evolving needs of patients
• Collaborate with interdisciplinary care teams to support patient rehabilitation goals

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Connecticut or eligibility for licensure
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility (SNF) setting

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with electronic medical records (EMR) systems used in SNF environments
