Physical Therapy Assistant (PTA) – Home Health

Portland, OR

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based (Home Health)

• Provide skilled physical therapy assistant services to patients in a home health setting across the West Portland area and surrounding communities
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC Remote Client for Home Health (laptop) and Rover mobile application
• Manage a daily patient caseload with travel between patient homes, with an occasional maximum radius of approximately 60 miles per day
• Deliver care in alignment with individualized patient treatment plans under the supervision of a licensed Physical Therapist

• Active Physical Therapy Assistant (PTA) license (pending license considered)
• Minimum 1 year of skilled home health experience required
• Basic Cardiac Life Support (BCLS) certification required
• COVID-19 primary vaccination series plus at least one booster required
• Reliable personal transportation required for daily patient visits

• Experience with OASIS documentation in a home health environment
• Familiarity with EPIC or similar electronic health record systems used in home health settings
• Comfort managing a geographically distributed patient caseload with varying daily drive times

• Mileage reimbursement provided at the IRS standard rate, with scheduling designed to minimize drive and windshield time
• No on-call, no required weekends, and no floating between units — offering a predictable and focused work schedule
