Key Responsibilities- Provide skilled physical therapy assistant services to patients in a home health setting across the West Portland area and surrounding communities
- Conduct up to 6 patient visits per day, meeting standard productivity expectations
- Document patient care using EPIC Remote Client (laptop) and Rover mobile application
- Manage daily scheduling and travel between patient homes, with occasional driving radius of up to approximately 60 miles per day
- Collaborate with the supervising physical therapist and care team to support individualized patient treatment plans
Required Qualifications- Active or pending Physical Therapy Assistant (PTA) license
- Minimum of 1 year of skilled home health experience preferred; first-time home health travelers considered
- Basic Cardiac Life Support (BCLS) certification required
- COVID-19 primary vaccination series plus one booster required
- Reliable personal transportation required for daily patient visits
- Experience with OASIS documentation
Preferred Experience / Skills- Familiarity with EPIC Remote Client and/or Rover charting platforms
- Comfort working independently in a field-based, community care environment
- Experience meeting productivity point expectations in a home health setting
